Hospice and family care services are tailored to meet the needs of those facing life-limiting illness. Our multidisciplinary team is dedicated to providing physical, emotional and even spiritual comfort to you, your loved one and the entire family.
What sets our program apart
Hospice & Family Care is committed to creating a space that has the comforts of home, no matter what location you choose. This means providing you with the subtle touches (favorite music, cherished photos) that bring you comfort – as well as surrounding you with family and friends to help you make the most of every moment. We will collaborate with you and your loved ones to determine which level of care is best for you.
We offer four levels of care:
- Routine care – we visit you in your home, managing symptoms and making sure you’re comfortable. We also educate your family on how best to care for you – including all medications, equipment and treatments.
- Continuous care – this is an alternative to going into the hospital for uncontrolled symptoms. Our team can provide up to 24-hour care for a short intermittent period of time to gain control of symptom burden.
- General inpatient care – when your symptoms are unable to be controlled in the home due to a higher level of care needed, admission to a hospital or skilled nursing facility for a short duration may be needed to gain control of symptoms.
- Respite care – when family members need an emotional or physical break, our team can offer a break from caregiving duties for up to five continuous days.
Legacy Wish Fulfillment
It is an honor to be given the privilege to serve you and your family by helping to support the end of life journey. Our team can assist patients in creating meaningful keepsakes that honor their legacy and help their families cope. You may also be interested in fulfilling a last wish that could help your loved one gain closure at their end of life. Our team will work closely with you and your family to discuss these projects and determine how we can help.

- Voice recordings
- Thumbprint pendants or framed
- Memory bears
- Photos with loved ones
- Written letters for future milestones (wedding, graduation, etc.)
We Honor Veterans
Our hospice agency takes part in a program known as We Honor Veterans, which creates guidelines and expectations for staff and community training to best care for our veterans. Our team is sensitive to the unique challenges veterans face when seeking medical care and promise to support you and honor your wishes every step of your hospice journey.
Hospice FAQs
Hospice is a comprehensive, holistic program of care and support for terminally ill patients and their families. Hospice care is care that focuses on managing pain and other symptoms, rather than curing q condition. It focuses on providing comfort and dignity to a person at the end of their life, while also providing emotional support. Our hospice services are available to patients of all ages, religions, races and illnesses.
Hospice care is usually covered under most private insurance plans, as well as Medicare, Medicaid, HMOs and other managed care organizations. You can check with our team to see if you are covered.
What’s the difference between hospice and palliative care?
Palliative care focuses on managing pain and discomfort and providing the highest quality of life alongside curative treatments. Hospice care also concentrates on pain management, but is provided when a cure is no longer possible.
Are there limits on who can receive hospice care?
Many people believe that they must be terminal, elderly or referred by a physician in order to receive hospice care. However, you or a loved one are able to access hospice services if you have any kind of life-limiting condition and meet eligibility criteria. You are able to leave hospice care whenever you want, and you can initiate the conversation about hospice care with your physician if you think it is the next step in your care.
